ADULT TREATMENT
& CASE MANAGEMENT SERVICES
ADULT CLINIC
The BHSN
Outpatient Adult Clinic
provides a full range of psychiatric services within a
clinic setting to Clinton County adults who have experienced
a serious mental illness. Many recipients are referred
following psychiatric hospitalization and are in need of an
array of treatment and rehabilitation services to fully
optimize their recovery. The Clinic is staffed by licensed
mental health professionals who provide assessment and
treatment services, verbal therapies, medication therapy and
education, health screening and referral, case management
and crisis interventions. The Adult Clinic emphasizes
medication management; brief solution focused and cognitive
behavioral therapies; crisis prevention; and case management
services. Clinic services are highly integrated with
rehabilitation and support activities as determined by
recipient’s individual service plan.
ASSISTED OUTPATIENT
TREATMENT (AOT) SERVICES/MEDICATION
GRANT PROGRAM
BHSN
acts as the AOT Coordinator for
Clinton County for court-ordered individuals needing
community supervision. AOT/Intensive Case Management
services are offered as a diversion from court-mandated
services. The Medication Grant Program provides medication,
at no charge, for individuals with a mental illness being
discharged from the jail or hospital while their Medicaid
application is pending.
CASE MANAGEMENT
Case Management provides support and
linkage services to adult recipients who are experiencing a
variety of difficulties in meeting their basic needs. Case
management services enable many clients to access mental
health services, medical care, housing, social services,
nutrition, and many other important community resources
necessary for their successful living in the community.
PROS (Personalized
Recovery Oriented Services)
The PROS Learning Center
is a complete recovery program that brings rehabilitation,
support and clinic services together into one plan that
supports participants’ ambitions and hopes. PROS services
are broken down into four categories of services with the
Individualized Recovery Plan keeping everyone working in the
same direction – helping an individual obtain a desired life
role.
CRISIS SERVICES
BHSN Crisis
Services provide 24-hour outreach support and intervention
to seriously mentally ill adults who are experiencing a
mental health crisis. Alternatives to hospitalization are
explored and linkages arranged where appropriate. This
service is frequently effective in providing individuals,
who may not be able to utilize office based services, with
the interventions and support which make hospital care
unnecessary. This program is highly effective at reducing
psychiatric hospitalizations by connecting individuals with
needed community outpatient services and resources.
INTENSIVE
& SUPPORTIVE CASE MANAGEMENT
ICM and SCM provide
intensive, flexible outreach linkage, support and
coordination of service to adults with serious emotional
disturbance. The recipients of ICM/SCM tend to be frequent
users of emergency services, at high risk of psychiatric
hospitalization and/or have not engaged in traditional
mental health services. A selection committee, comprised of
various community service providers, refers individuals to
the program. ICM/SCM services allow many recipients to
minimize psychiatric hospitalizations by engaging them in
needed community services.
