CHILD & FAMILY SERVICES

INTENSIVE CASE MANAGEMENT (ICM)/ SUPPORTIVE CASE MANAGEMENT (SCM)

Intensive Case Management (ICM) provides intensive, flexible outreach linkage, support and coordination of service to children with serious emotional disturbance.

The recipients of ICM/SCM tend to be frequent users of emergency services, at high risk of psychiatric hospitalization and/or have not engaged in traditional mental health services. A selection committee, comprised of various community service providers, meets monthly to determine whether a referral is appropriate for ICM/SCM or another community-based case management program. Referrals may be made by: schools, private practitioners, parents, DSS or other agencies.

To obtain information please call 518-561-1767 or click here.  ICM/SCM services allow many recipients to minimize psychiatric hospitalizations by engaging them in needed community services.
