COMMUNITY RESIDENCE SUPERVISOR
Breakthrough II is seeking a full-time Supervisor to provide programmatic & fiscal oversight of a community residence designed to assist adults with mental illness learn the skills necessary to live independently. Responsibilities include: coordinate and reinforce integrated residential, rehabilitation and clinical treatment of residents; monitor work quality and quantity of assigned staff and facility outcomes. Participate in the screening, hiring, orienting and training of new employees; and ensure implementation of Agency regulatory policies and procedures. Qualifications include: Bachelor’s degree in Human Services related field or Associate's Degree in Human Services related field and 3 years exp. in human services. Three years prior exp. in a supervisory capacity preferred. Understanding of OMH regulations preferred. Excellent communication skills, both written and verbal needed. Strong supervision, time management, problem solving and computer/keyboarding skills are essential.
COMMUNITY ADVOCATE (Franklin County)
The STOP Domestic Violence program is seeking a full-time Advocate in Franklin County. Duties include: provide case review, transportation, advocacy, safety planning, assistance with petitions and follow-up as necessary. Bachelor’s degree in appropriate field preferred. Will act as liaison with community resources. Excellent communication skills and ability to work with minimum supervision essential. Valid NYS driver’s license and reliable transportation required.
HOMESTEADS ON AMPERSAND SUPPORT STAFF (HOA)
BHSN is seeking relief Support Staff to assist adults with mental illness in permanent supportive living settings. Position entails assisting with daily living skills, advocating for clients, developing individualized housing assessments and support plans to insure clients are connected to appropriate supports in the community. Good interpersonal, organizational, computer and data entry skills required. GED/HS Diploma required; Associates degree in relevant Human Services field with 2 yrs. employment experience in a helping capacity and/or certification of peer support training preferred. Reliable transportation and valid NYS driver’s license for required travel.
BHSN is seeking a full-time NYS Registered Nurse to provide health monitoring and screening, coordination of health referrals and work in close collaboration with prescribers at the Adult and Child/Family Clinics. The RN will work closely with prescribers to support medication treatment services, including administration and management of prescription medications, to include injectables, and manage a client caseload, including development of treatment plans. Prior experience working in a mental health environment preferred. Some evening hours required. Computer and keyboard skills for using electronic health record, CPR certification, and valid NYS driver’s license acceptable to BHSN insurance carrier all required.
BHSN is looking for a School-Based Clinician to provide evidence based individual, group and family interventions entirely in school settings. This is a full time position, following a modified school calendar. Primary responsibilities include engaging child/family in a comprehensive assessment of strengths/needs of a child/family and developing treatment plans based on assessment of need. Will also provide individual/group/family therapies and facilitate crisis intervention services as clinically needed and have the ability to coordinate services in different schools. Qualifications include: NYS LMHC, LMSW, or LCSW. Previous experience working in a school setting preferred. Must be able to work flexible hours to meet the needs of children and families. Travel between schools required.
RESIDENTIAL AIDE (Twin Oaks)
Twin Oaks is seeking Residential Aides on a part-time and relief basis to assist residents in maintaining sobriety/abstinence skills as they transition toward a drug-free lifestyle. Duties may include: assisting counselors, providing client supervision in a residential setting, recordkeeping, medication supervision and accountability and monitoring of daily activities as necessary. Those with experience and knowledge of alcohol and substance abuse delivery systems are encouraged to apply. Experience in substance abuse field is preferred. Training is provided. Applicants must possess a valid driver’s license acceptable to BHSN’s insurance carrier. Some local travel may be required. Twin Oaks is a tobacco-free environment.
APARTMENT PROGRAM SUPERVISOR

Strong supervisory, organizational and interpersonal skills are necessary. Associates degree in related field and 3 years’ experience in human services required. BA in relevant human services field and experience working with adults with mental illness or co-occurring disorders preferred. Applicants must possess a valid NYS driver’s license acceptable to BHSN insurance carrier. Some local travel may be required.
RESIDENTIAL RELIEF STAFF (Various Shifts)
Breakthrough II, a community residence for adults, is seeking dependable and responsible adults for Relief positions. Duties include assisting counselors, providing client supervision in residential setting, record keeping and monitoring daily activities as necessary. Candidates must possess a high school diploma or equivalent; relevant experience in human services field preferred. Valid NYS driver’s license acceptable to BHSN's insurance carrier required. CPR certified or able to achieve certification within 6 months of hire. Basic computer and keyboarding skills essential. Must be able to climb/descend stairs, perform light household indoor and outdoor chores, and react quickly to situations inside and outside the residence. Hours are scheduled on an as-needed basis. All shifts are stay-awake shifts.
PEER DRIVER (PROS)
The PROS program is seeking a qualified Peer Driver to provide transportation assistance to help individuals keep wellness appointments. Drivers track daily mileage of transports, perform maintenance checks of vehicles, and insure that all required regulatory paperwork is completed. Valid NYS driver’s license acceptable to BHSN insurance carrier required. Must maintain an excellent driving record as a requirement of continued employment. High School diploma or equivalent is required.
HOME VISITOR (Healthy Families NY – Franklin County)
Full-time Home Visitor’s are needed to assist pregnant women and newly parenting families in Franklin County. Home Visitors provide strength based support and advocacy utilizing program resources including parenting curriculum and standardized assessment tools. Need for flexible work hours to include evenings and possibly weekends to accommodate families’ needs. Healthy Families – Franklin is an accredited Healthy Families New York Home Visiting Program. Applicants should possess at least a H.S. diploma, experience in a Health & Human services field and/or child care, strong organizational, time management and writing skills essential. Valid NYS driver’s license acceptable to BHSN insurance carrier with reliable transportation required.
CLINICIAN (Adult Clinic)
BHSN is seeking a F/T Clinician for its outpatient Adult Clinic. Responsibilities include comprehensive assessment, treatment planning, and individual/group therapy. Will include evening hours as assigned. NY State licensure in Social Work (LCSW) required; LCSW-R preferred.
DRIVER (P/T) (Transportation)
BHSN seeks part-time Drivers to transport people to and from our Morrisonville, NY, location. Morning and afternoon shifts available. Valid NYS Driver’s license acceptable to BHSN's insurance carriers required.
