COMMUNITY RESIDENCE SUPERVISOR

Breakthrough II is seeking a full-time supervisor to provide programmatic & fiscal oversight of a community residence designed to assist adults with mental illness learn the skills necessary to live independently. Responsibilities include: coordinate and reinforce integrated residential, rehabilitation and clinical treatment of residents; monitor work quality and quantity of assigned staff and facility outcomes; participate in the screening, hiring, orienting and training of new employees; and ensure implementation of Agency regulatory policies and procedures.  Qualifications include: Bachelor’s degree in Human Services related field or Associate's Degree in Human Services related field and 3 years exp. in human services.  Three years prior exp. in a supervisory capacity preferred.   Strong supervision, time management, communication, problem solving and computer/keyboarding skills are essential. 

 

HOME VISITOR (Healthy Families NY)

Full-time Home Visitor is needed to assist pregnant women and newly parenting families in Clinton County.  Home Visitors provide strength based support and advocacy utilizing program resources including parenting curriculum and standardized assessment tools.  Need for flexible work hours to include evenings and possibly weekends to accommodate families’ needs.  Healthy Families is an accredited Healthy Families New York Home Visiting Program.  Applicants should possess at least a H.S. diploma, experience in a Health & Human services field and/or child care, strong organizational, time management and writing skills essential.  Valid NYS driver’s license acceptable to BHSN insurance carrier with reliable transportation required.

 

ADVOCATE (Essex County)

The STOP Domestic Violence program is seeking a full-time Advocate in Essex County. Duties include: providing supportive counseling, transportation, advocacy, safety planning, safe housing, child care and assistance filing court petitions as necessary.  High School diploma or equivalent required, bachelor’s degree in appropriate field preferred.  Excellent communication skills and ability to work with minimum supervision essential. Valid NYS driver’s license acceptable to BHSN’s insurance carrier and reliable transportation required.

 

BILLING CLERK

A Billing Clerk is needed to provide data entry, filing, and telephone support to program staff. Prior experience in an office setting, strong/accurate data entry and communication skills essential. Must be detail-oriented and able to work independently. Proficiency with Excel spreadsheets preferred. High School Diploma or equivalent required. This is a temporary position with the potential for permanent employment.

 

CLINICIAN (Full-Time)

CRISIS CLINICIAN

The mobile crisis team is seeking a full-time Crisis Clinician to provide crisis intervention in the community and/or office setting during weekday afternoons and evenings as a member of the mobile crisis team.  Responsibilities would include making risk assessments, providing brief interventions, care management and individual/family psychotherapy to individuals potentially in crisis.  Qualified candidates must possess either an LMHC, LMHC-P, LCSW, LCSW-R, or an LMSW.  Travel throughout Clinton County is necessary.  A valid NYS driver license acceptable to BHSN insurance carrier is required.

 

CHILDREN’S CARE MANAGER

Health Home Serving Children is seeking a full-time individual to provide care coordination services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program.  The Care Manager gathers assessments and enters documentation that confirms eligibility and identifies areas requiring supportive services, works with children, families and service providers to create a comprehensive Plan of Care, identifies additional service providers as necessary, and coordinates the continuing involvement of families and service providers in the execution of the plan, then documents progress and arranges for eventual transition between health homes as necessary, or from health homes to other levels of service following changes in eligibility.  A bachelor’s degree in human services field with 2 years’ relevant experience or Master’s degree in human services field with 1 year of relevant experience required.  Excellent engagement and organizational skills as well as ability to work independently and collaboratively are a must. Valid NYS driver license acceptable to BHSN insurance carrier required.

 

CLINICAL COORDINATOR

The Adult Outpatient Clinic is seeking a full-time Clinical Coordinator to provide clinical supervision and oversight to social workers and licensed mental health counselors within the clinic setting.  This position also provides individual and group therapy to adults.  3 years’ experience in providing psychotherapy as well as 1-year supervisory experience within a clinical setting are required.  Qualifications include the following licensures: LMHA, LMSW, LCSW, or LCSW-R.  LCSW-R is strongly preferred. 

 

FAMILY PEER ADVOCATE

The Families Involved Together (FIT) program of BHSN is seeking a parent, foster parent or other individual with experience raising a child with psychiatric, emotional or behavioral challenges, to provide family support services on a part time basis.  Responsibilities include: providing support and advocacy to parents of identified children to include development of wraparound plans, resource acquisition and coordination of service plan implementation.  Qualified candidates must have a High School diploma or equivalent.  A bachelor’s degree in a Human Service related field is preferred. Strong communication and engagement skills, flexible work schedule, a valid NYS driver’s license, and reliable transportation are required.

 

ACCOUNTING CLERK

BHSN is seeking a full-time Accounting Clerk for our Finance Department.  A successful candidate will be responsible for assisting with Accounts Payable, Purchasing, Payroll, claiming funds from local and state government, and other administrative support.  Attention to detail is necessary. Applicants must have a minimum of an Associate’s Degree in Accounting or Business/Finance related field, a bachelor’s degree is preferred. 

 

ADDICTION COUNSELOR (Twin Oaks)

Twin Oaks, a community residence for individuals in recovery, is seeking a Full-time Addiction Counselor to assist residents in maintaining sobriety/abstinence skills as they transition toward a drug-free lifestyle. Applicants must have experience and knowledge of alcohol and substance abuse delivery systems. A HS diploma or equivalent is required. CASAC or CASAC-T is preferred, as a CASAC-T must be obtained within 2 years. Experience in substance abuse field, engagement skills, and IT skills are necessary.  Qualified candidates must possess a valid driver’s license acceptable to BHSN’s insurance carrier. Some local travel may be required. Twin Oaks is a tobacco-free environment.

 

RESIDENTIAL AIDE (Twin Oaks)

ADULT CARE MANAGER (Adult Care Management)

Working within the Health Home, the Adult Care Management division of BHSN is seeking a full-time individual to organize client care in order to improve clients’ health outcomes and reduce unnecessary emergency room and inpatient hospitalizations. The Care Manager will assess clients’ needs; develop and implement comprehensive care plans; and coordinate personnel and resources needed to facilitate the appropriate delivery of health care services. They will also document all services and conduct care coordination meetings as clinically indicated and in conjunction with regulatory policies and procedures. Required: two years’ experience working directly with persons with behavioral health diagnosis and either a bachelor’s level education or higher, a NYS licensed RN with a bachelor’s degree, or a Credentialed Alcoholism and Substance abuse Counselor (CASAC). Excellent engagement and writing skills are essential.  Valid NYS driver’s license and CPR/Safety training certification needed. 

 


COMPLIANCE SPECIALIST

BHSN is seeking a full-time Compliance Specialist, specializing in billing compliance, to ensure the agency is operating in accordance with regulatory standards, particularly OMH and OASAS.  The Compliance Specialist would be expected to conduct audits on clinical records and program documentation, participate in regulatory certification processes, and be familiar with federal, state and other governing agency regulations.  A bachelor’s degree, familiarity with CPT 4 codes and ICD 10, and prior experience in medical records are preferred.  HS Diploma or equivalent with a minimum of 2 years of billing compliance required.

 

RESIDENTIAL SUPPORT STAFF (various shifts) 

Breakthrough II, a community residence for adults, is seeking dependable individuals for full-time, part-time and relief positions. Duties include assisting other support staff to provide client supervision in residential setting, record keeping and monitoring daily activities as necessary. Candidates must possess a high school diploma or equivalent; relevant experience in human services field preferred. Valid NYS driver’s license acceptable to BHSN's insurance carrier required.   CPR certified or able to achieve certification within 6 months of hire. Basic computer and keyboarding skills essential. Must be able to climb/descend stairs, perform light household indoor and outdoor chores, and react quickly to situations inside and outside the residence. Breakthrough II is a 24-hour program.  Overnight shifts will be necessary.  All shifts are stay-awake shifts.

 

CHILDREN SERVICES COORDINATOR (Franklin County)

The STOP Domestic Violence program is seeking a full-time Coordinator in Franklin County to provide services for children whose parent(s) are clients at STOP DV. Duties include: meeting with children living in safe dwellings to provide a safe and nurturing environment, provide transportation, advocacy, safety planning and follow-up as deemed appropriate, as well as participating in back-up coverage to volunteers on the hotline during non-business hours. Excellent communication skills and ability to work with minimum supervision essential. Valid NYS driver’s license and reliable transportation required.

 

HOMESTEADS ON AMPERSAND SUPPORT STAFF (HOA)

BHSN is seeking relief Support Staff to assist adults with mental illness in permanent supportive living settings.  Position entails assisting with daily living skills, advocating for clients, developing individualized housing assessments and support plans to insure clients are connected to appropriate supports in the community. Good interpersonal, organizational, computer and data entry skills required. GED/HS Diploma required; Associates degree in relevant Human Services field with 2 yrs. employment experience in a helping capacity and/or certification of peer support training preferred. Reliable transportation and valid NYS driver’s license for required travel.

 

REGISTERED NURSE

BHSN is seeking part-time and full-time NYS Registered Nurses to provide health monitoring and screening, coordination of health referrals and work in close collaboration with prescribers at the Adult and Child/Family Clinics.  The RN will work closely with prescribers to support medication treatment services, including administration and management of prescription medications, to include injectables, and manage a client caseload, including development of treatment plans.  Prior experience working in a mental health environment preferred. IT skills for working with electronic health records, CPR certification, and valid NYS driver’s license with driving record acceptable to BHSN insurance carrier all required.

 

CLINICIAN (School-Based)

BHSN is looking for a School-Based Clinician to provide evidence based individual, group and family interventions entirely in school settings. This is a full time position, following a modified school calendar. Primary responsibilities include engaging child/family in a comprehensive assessment of strengths/needs of a child/family and developing treatment plans based on assessment of need. Will also provide individual/group/family therapies and facilitate crisis intervention services as clinically needed and have the ability to coordinate services in different schools. Qualifications include: NYS LMHC, LMSW, or LCSW. Previous experience working in a school setting preferred. Must be able to work flexible hours to meet the needs of children and families. Travel between schools required. 

 

APARTMENT PROGRAM SUPERVISOR

BHSN is looking for a Supervisor to provide fiscal and programmatic oversight of an 18 bed Apartment Program designed to assist adults with mental illness learn the skills necessary to live independently.  Strong supervisory, organizational and interpersonal skills are necessary. Associates degree in related field and 3 years’ experience in human services required.  BA in relevant human services field and experience working with adults with mental illness or co-occurring disorders preferred. Applicants must possess a valid NYS driver’s license acceptable to BHSN insurance carrier.  Some local travel may be required.

 

PEER DRIVER (PROS)

The PROS program is seeking a qualified Peer Driver to provide transportation assistance to help individuals keep wellness appointments.  Drivers track daily mileage of transports, perform maintenance checks of vehicles, and insure that all required regulatory paperwork is completed.  Valid NYS driver’s license acceptable to BHSN insurance carrier required.  Must maintain an excellent driving record as a requirement of continued employment.  High School diploma or equivalent is required. 

 

DRIVER (P/T) (Transportation)

BHSN seeks part-time Drivers to transport people to and from our Morrisonville, NY, location. Morning and afternoon shifts available. Valid NYS Driver’s license acceptable to BHSN's insurance carriers required.

To help you understand the terms Relief, Part-time or Full-time above: a Relief staff member works on an "as needed" basis; a Part-time staff member usually works less than 40 hours per week depending on the program/job description. A Full-time staff member is someone who normally works at least 40 hours per week.
