Adult HCBS provides a flexible and recovery-oriented framework for adults (ages 21 and over) to improve their quality of life. Ages Served
Individuals 21 years old and over.
Eligibility
Individuals with a history of Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD) who are experiencing challenges in daily life.
Insurance & Payment
Must be enrolled in Managed Medicaid Insurance with a HARP (Health and Recovery Plan).
