Program Overview

Adult HCBS provides a flexible and recovery-oriented framework for adults (ages 21 and over) to improve their quality of life. This program is built on the principle that your voice and your choices come first. Our dedicated team works alongside you to develop a customized plan that focuses on skill-building and resource-connection.   • Habilitation Services: We partner with you to build the essential skills for daily living and independence. This includes hands-on support with budgeting, mastering daily routines, and learning how to comfortably navigate and access community resources.


  • Comprehensive Employment Support: Whether you are looking for your first job or returning to the workforce, we are here to help. We assist with identifying your career interests, researching options, applying for roles, practicing interview techniques, and providing ongoing support once you’ve started your new position.


  • Education Support: If you’re looking to further your education, we provide the guidance needed to identify your academic goals and navigate the application process. We also offer coping skills and practical support to help you stay successful and balanced while you complete your courses.


  • Goal-Oriented Recovery: Our services are designed to help with relapse prevention and personal growth, ensuring you have the tools to maintain your wellness.

Program Qualifications

Ages Served

Individuals 21 years old and over.

Eligibility

Individuals with a history of Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD) who are experiencing challenges in daily life.

Insurance & Payment

Must be enrolled in Managed Medicaid Insurance with a HARP (Health and Recovery Plan).
