Community Oriented Recovery and Empowerment
Recovery is a journey, and our CORE Services are designed to meet you right where you are: in your home and community. The program provides flexible, person-centered support to adults navigating serious mental health and/or substance use challenges, helping them build the skills and confidence necessary to live a fuller, more independent life.

Program Overview
CORE is a home and community-based rehabilitative service for adults (age 21 and older) with a history of serious mental illness (SMI) and/or substance use disorders. Our recovery-oriented approach focuses on empowerment, helping individuals improve their quality of life, strengthen new skills, and achieve their personal goals, such as finding employment, pursuing education, or preventing relapse.
Getting Started:
CORE Services has a “No Wrong Door Pathway” referral process. This means that anyone can refer to the program including mental health professionals, doctors, nurses, care managers, family, and friends. Self-referrals are accepted as well! If individuals qualify for services and wish to apply, they must complete this referral form and email it to our team at core@bhsn.org.

Services Offered
Our multidisciplinary team delivers four key recovery-focused services in the home of the individuals we serve or community:
Psychosocial Rehabilitation Services: Services designed to help the individuals we serve rebuild skills that may have been disrupted by their mental health or substance use disorder. This includes developing social skills, coping mechanisms, and successfully engaging with the community.
Peer Empowerment Services: Providing invaluable support, mentorship, and advocacy from individuals who have lived experience with recovery.
Community Psychiatric Supports and Treatment (CPST): Providing individual psychotherapy in the home of the individuals we serve or community setting, which is particularly helpful for those who find it challenging to engage in traditional agency-based therapy.
Family Support & Training Services: Focusing on improving communication, understanding, and relationships within the family unit by educating family members about the mental health or substance use disorder of the individuals we serve.
