Program Overview

Our Breakthrough II  Apartment Program offers a less intensive level of care for persons recovering from mental illness as participants reside in either 1 or 2 bedroom apartments located within the City of Plattsburgh. All residents have their own bedrooms, however, those living in 2 bedroom units share common space such as kitchen, living room and bathroom. Case Managers meet with residents throughout the week to offer rehabilitative support services.

Services Offered

  • Independent Living with a Safety Net: Reside in comfortable one- or two-bedroom apartments within the City of Plattsburgh, ensuring you stay connected to your community while having a stable place to call home.


  • Personalized Case Management: You won't navigate this journey alone; our Case Managers meet with you throughout the week to provide rehabilitative support and guidance tailored to your goals.


  • Private and Shared Spaces: Every participant enjoys the privacy of their own bedroom. In our two-bedroom units, you’ll have the opportunity to build social connections in shared common areas like the kitchen and living room.


  • Skill Building: Focus on the practicalities of daily life (from budgeting to wellness) with a team dedicated to helping you maintain your independence.

Program Qualifications

Ages Served

Individuals 18 years old and up.

Eligibility

Individuals with a Serious Mental Illness (SMI) diagnosis as defined by OMH (Office of Mental Health).

Insurance & Payment

Medicaid insurance is required for this program.
