The Breakthrough II Community Residence is a co-ed congregate care level II facility that provides 24/7 on-site support for individuals recovering from mental illness who require a structured living environment. We focus on intensive restorative services tailored to each individual's needs and readiness to participate. The residence features two specialized "practice apartments" designed to help residents master independent living skills while still benefiting from a supportive, structured environment before transitioning to a lesser level of care.
