We don't provide direct medical treatment; instead, we provide the coordination, advocacy, and strategic planning necessary to improve your quality of life. Whether you are an adult managing chronic conditions or a parent seeking support for a child with complex needs, our team works to reduce hospital visits and increase your sense of control over your own health.

While the goals are the same, we offer specialized distinctions based on location and age:
Available in Plattsburgh & Queensbury
Comprehensive Care Planning: We perform a deep-dive assessment of medical, behavioral, and social needs to build a personalized plan reflecting your preferences.
Health Promotion & Advocacy: Your Care Manager links you to doctors and specialists, ensuring you get the right care at the right time. We monitor your progress and "course correct" as your needs change.
Transitional Care: If you or your child are being discharged from a hospital or facility, we facilitate the planning to ensure the transition home is safe and supported.
Referral to Community Supports: We identify and connect you to essential local resources, including food benefits, transportation, and peer support groups.

Adult Care Management (Plattsburgh Only): Dedicated support for adults (21+) with Serious Mental Illness (SMI) or multiple chronic conditions. We focus on stabilizing health outcomes and securing stable housing or social benefits.
Children’s Care Management (Plattsburgh & Queensbury): Focused on youth (0–21) with Serious Emotional Disturbance (SED) or complex trauma. In Queensbury, we specifically prioritize helping families navigate school-based systems and developmental supports.

Plattsburgh: Youth: 0–21; Adults: 21+ (or 18+ if living independently/parenting).
Eligibility
Plattsburgh: Diagnosis of Serious Mental Illness (SMI) or two chronic conditions (e.g., Asthma, Diabetes, Substance Use).
Queensbury: Diagnosis of Serious Emotional Disturbance (SED), Complex Trauma, or two chronic conditions.
Insurance & Payment
Plattsburgh: Active Medicaid (Limited non-Medicaid slots available via SPOA Committee).
Queensbury: Active Medicaid required.
