Welcoming a new baby is an exciting time, but it can also bring new challenges and stress. Our Healthy Families North Country program provides personalized, in-home support to help you build the strongest possible foundation for your child's well-being and your family's success. We believe that a positive parent-child relationship, even before birth, is the key to healthy development.

Program Overview

Healthy Families is a voluntary and free home-visiting program designed to support expectant and new parents. Once enrolled, a family receives visits from a highly trained Home Visitor for 3 to 5 years.


Our services are research-based and focus on creating a fun, interactive, and supportive experience. We work with you to develop an individualized family goal plan, connect you with community resources, and empower you with the skills you need to navigate parenthood successfully.

Services Offered

Our program is designed to support the entire family, focusing on building a positive and healthy relationship between parents and their children.


  • Personalized Home Visits: Services are delivered through scheduled home visits, including virtual visits, to meet the needs of mothers and fathers.

  • Developmental Screenings: Infants receive regular developmental screenings to ensure they are meeting milestones and to catch any potential concerns early.

  • Resource Navigation: Home visitors offer referrals to appropriate community resources based on the specific needs expressed by the family.

  • Goal Planning: Support is provided in creating an individualized family goal plan and identifying strong support systems for the family.

  • Child Welfare Support: We welcome referrals for parents with children up to 24 months of age when the family is actively involved with the Department of Social Services (DSS - Preventive, CPS, or Foster Care), utilizing specialized child welfare protocols to engage the family.

Ages Served

Expectant mothers and fathers (pregnant) or families with a newborn baby under 3 months old. The program can also accept referrals from DSS (preventive, CPS, or Foster services) for parents with children up to 24 months of age.

Eligibility

Must be an expectant mother/father or have a newborn under 3 months old. Families involved with DSS might be eligible beyond the typical intake if referred by DSS for a child up to 24 months.
